What to Expect?

When you call our office to make an appointment, Robin or April will help you find a convenient time to come in. They will ask for your phone number, address, date of birth, social security number if needed for insurance, and any insurance information so that our office can better help you know what your insurance will cover according to your dental needs.

A new patient information form and HIPAA information will be mailed to you along with a welcome letter after your appointment is arranged on the phone. Once you receive the forms, fill them out and bring them with you to our office on the day of your appointment. Be sure to bring any list of medications you are taking with your forms.

Once you come in the office, all of our staff will meet you.

On the Patient Forms page, you can click on the forms and email them to us if you prefer. You also have the option to print them out and bring them with you to the office.
